Preferred Provider Organization (PPO)
You retain the freedom to choose any medical provider at the time you or your dependents need medical care. Benefits are paid at 90% if you choose an in-network provider, and 70% if you choose an out-of-network provider. After reaching your annual out-of-pocket maximum, either in-network or out-of-network, benefits are payable at 100%.

Health Maintenance Organizations (HMOs)
Medical care must be arranged through your personal Primary Care Physician. You may choose a different Primary Care Physician for yourself and each member of your family. Benefits are paid at 100%, although a small co-payment is required for some services. There are no deductibles, maximums or claim forms.

Optional Life Insurance allows you to purchase additional life
insurance in the following amounts:
- Employee: up to five times annual base salary (see limit above)
- Spouse: up to a maximum of $100,000
- Children: $5,000 to $25,000
- Personal Accident Insurance provides you with the opportunity to obtain additional accidental death and dismemberment insurance for you and your eligible dependents. Coverage may be elected in amounts up to six times your annual base salary for yourself (see limit above), up to 50% of your coverage for a spouse, and lesser of 20% or $10,000 for children
